Sports Physicals

Summer Sports Physical Clinics only $25*

Providers from Uinta Medical Group will be offering physicals 5/28 & 5/29.

When: May 28, 2026 from 9:00 am - 3:00 pm
Where: Evanston High School Gym (enter through the front doors during school hours)
Evanston Regional Hospital will be there free optional pre-injury screenings.
*For each physical provided at this event. $10 will be donated back to the Uinta County School District #1.

When: May 29, 2026 from 8:00 am - 12:00 pm
Where: Evanston High School Gym (enter through the Jock Lot enterance)
Evanston Regional Hospital will be there free optional pre-injury screenings.
*For each physical provided at this event, $10 will be donated back to the Uinta County School District #1.

When: August 12, 2026 from 9:00 am - 3:00 pm
Where: Davis Middle School
Evanston Regional Hospital Physical Therapy Department will be there for free optional pre-injury screenings.
*For each physical provided at this event, $10 will be donated back to the Uinta County School District #1.

When: August 13, 2026 from 9:00 am - 3:00 pm
Where: Evanston Middle School
Evanston Regional Hospital Physical Therapy Department will be there for free optional pre-injury screenings.

Dr. Richard England will be providing the sports physicals. Evanston Regional Hospital's Physical Therapy Team will provide FREE Optional Pre-Injury Screenings.

Students will need to bring completed consent forms (download from the buttons below) along with the UCSD#1 Physical Exam form which can be found on the school’s website or by following this link.

Consent Forms:

Consent forms are required upon check-in. The Evanston Regional Hospital (ERH) Consent Form gives permission for the ERH Physical Therapist to complete the 10-minute pre-injury screening which is designed to define a student’s movement baseline. The Uinta Medical Group consent form is required prior to your student being seen for the sports physical exam.

School Donation:

For every person taking advantage of the $25 Cash Pay special for sports physicals completed a $10 donation will be made to UCSD#1. However, if an insurance plan is billed for the sports physical, neither the $25 price nor the $10 school donation can be honored.

Insurance:

Most insurance companies do not pay for sports physicals in the same manner they pay for a wellness physical. Be sure to check with your provider if you have questions about coverage. The $25 price and the ability to offer a rebate to the school are for cash-pay patients only. If insurance is billed for a sports physical, we are regretfully unable to offer the cash-pay price or apply the rebate to the school.

What is a sports physical?

A sport physical is a non-invasive exam that helps healthcare providers determine if the athlete should participate in a specific sport. The provider may look at the patient's medical history and current overall health to make this type of determination. Keep in mind that a sports physical is not the same as a wellness physical. While a sports physical looks for health conditions that could impact the ability of the athlete to play a sport, it doesn’t look at all possible health issues. It is important that students maintain annual exams with primary care providers.

More and more often, sports physicals are required for many sports in both club and school level teams. Physicals are typically good for one (1) year, meaning most sports will require a new physical each season. Please check with your school guidance counselors for more information on which extra curricular activities or clubs may require a sports physical.
